SVF Stem Cell Therapy

SVF stem cell therapy is one of the most advanced applications of regenerative medicine. It uses the stem cells and repair cells hidden in your own fat tissue to support tissue repair, slow visible ageing and help the skin renew itself naturally. SVF stands for “stromal vascular fraction”. A small amount of fat is taken from your abdomen or hips, processed to separate its rich population of stem cells and other repair cells, and this concentrated cell mixture is injected back where it is needed.

Unlike other stem cell applications, it needs no laboratory cell multiplication: the whole process can be completed in a single session within a few hours. At our clinic in Beşiktaş, Istanbul, I offer it to international standards with an approach tailored to each patient. The aim is not a temporary fix with synthetic substances but a long-lasting, natural improvement using your own biological resources.

A Treasure in Fat Tissue: What Is SVF?

For years, fat tissue was seen only as an energy store or unwanted excess. Research has shown that it is in fact one of the body’s richest sources of stem cells – a “regenerative treasure”. SVF is the purified, concentrated form of this resource: all the cells in fat tissue other than the fat cells themselves (adipocytes). When fat taken by mini-liposuction is processed enzymatically or mechanically, the fat cells are separated and a rich cell mixture from the tissue’s supporting structure (stroma) and blood vessels (vascular) remains. Its value lies in its variety of cells working together:

  • Mesenchymal stem cells (MSCs): the most important component, able to develop into many cell types such as bone, cartilage, muscle, fat and skin cells – the managers of regeneration.
  • Endothelial progenitor cells (EPCs): trigger new blood vessel formation (angiogenesis), improving nourishment, oxygen supply and waste removal in the treated area.
  • Preadipocytes: precursors of fat cells that help restore facial volume for a natural fullness.
  • Fibroblasts: produce collagen and elastin, improving firmness and elasticity.
  • Immunomodulatory cells (such as T-reg cells and M2 macrophages): regulate inflammation so that repair proceeds in a healthier, more organised way.

So SVF provides not just one cell type but also the vessel builders, protein producers and regulating cells that create the right environment for stem cells to work – a broader regenerative effect than treatments containing only growth factors (such as PRP) or a single cultured cell type.

SVF Stem Cell Therapy

SVF stem cell therapy is the re-injection of a rich cell concentrate from your own fat tissue – containing mesenchymal stem cells and millions of repair cells – for aesthetic purposes such as skin rejuvenation, hair loss and scar treatment. It works on a “point-of-care” principle, with fat harvesting, cell separation and injection all completed in about 2–3 hours, based on the discovery that fat tissue is a richer source of mesenchymal stem cells than even bone marrow. It is performed under sterile conditions in three stages:

  1. Fat harvesting (mini-liposuction): usually from the abdomen, waist or hips. After local anaesthetic, about 50 to 100 cc of fat (roughly a tea glass) is gently taken through a tiny incision of a few millimetres with special cannulas. It is quick, comfortable and causes minimal trauma, unlike large cosmetic liposuction; the incision usually needs no stitches.
  2. Isolating the SVF: in a closed, sterile system, the fat is broken down mechanically and then separated from the SVF using enzymes such as collagenase or special filtration. The fat cells are discarded, leaving a concentrated fluid containing millions of stem cells, progenitor cells, fibroblasts and other repair cells. This takes about 60–90 minutes without contact with the outside environment.
  3. Injection: the fresh SVF concentrate is drawn into syringes and injected immediately. For facial rejuvenation it is usually applied across the face with mesotherapy, fine needles or a cannula, and more intensively in areas of volume loss or deep wrinkles. For hair loss, it is injected into the scalp to nourish the follicles directly.

Uses in Aesthetic Medicine

Skin rejuvenation and anti-ageing: the most popular use, for volume loss, deep wrinkles, reduced elasticity and declining skin quality. Mesenchymal stem cells and fibroblasts stimulate new collagen and elastin, so the skin becomes firmer and fuller over the following months; endothelial progenitor cells improve circulation for a brighter, healthier colour; preadipocytes can restore facial volume naturally, a kind of “biological filler” using your own tissue; fine lines soften, pores look tighter and texture becomes smoother.

Scars and acne scars: thanks to its repairing properties, SVF can also be used for atrophic acne scars, surgical scars and burn scars, helping depressed areas fill and scarred tissue look more like normal skin.

Hair loss: injected into the scalp, SVF can revitalise the tissue around the follicles, improve circulation and may stimulate dormant cells, helping existing hair thicken and slowing shedding.

The potential of SVF is also being researched outside aesthetics, for example in orthopaedics and chronic wound healing. At our clinic, the focus is on aesthetic uses.

The Process, When Results Appear and Safety

Although advanced, SVF is comfortable for the patient and completed in one day, usually 2 to 3 hours:

  • Consultation: a detailed examination and discussion of your expectations, medical history and suitability.
  • Fat harvesting: under local anaesthetic, so you feel no pain; about 20–30 minutes.
  • Cell separation: about 1–1.5 hours in closed-system devices.
  • Injection: immediately afterwards, with fine needles or a cannula; about 20–30 minutes.

After a short rest you can return to daily life. The harvest site may be slightly tender or bruised for a few days, usually managed with simple painkillers.

SVF does not give instant volume like a filler (though some fullness may be seen if fat is also injected). It is a biological repair process: the injected cells need to settle, activate and start producing new collagen, elastin and blood vessels. First effects – more vitality, radiance and hydration – usually appear within 1–2 months. Noticeable results such as firmer skin, fewer fine lines and better quality usually appear between 3 and 6 months, and the full effect can take up to a year to settle. Because it supports the skin at a cellular level, results are long-lasting and often continue for several years, though this varies between individuals.

Safety: because only your own tissue is used, the risk of allergic reaction or rejection is extremely low. With sterile conditions and closed systems, the risk of infection is minimal; the only external substances are the enzymes used to break down the fat, which are then removed. It is a good option for people looking for natural, biological treatment and willing to be patient for gradual results.
