Chin Filler

Chin filler is a modern, effective, non-surgical procedure to improve the shape, size and projection of the chin (pogonion), one of the most important elements of the profile and of facial aesthetics overall. A strong chin in harmony with the other features balances the profile and gives the face a more defined, attractive and confident expression. A chin that is naturally receding, small or asymmetrical can disrupt facial harmony, make the nose look bigger and make the area under the chin look fuller.

Chin filler offers a comfortable, non-surgical solution with immediate results that respects the face’s natural proportions. It uses hyaluronic acid fillers with the density and lifting capacity to imitate bone. At our clinic in Beşiktaş, Istanbul, I assess each patient’s face holistically, almost like a sculptor. The aim is not just to enlarge the chin but to balance it with the nose, lips and jawline. For those who prefer to avoid the risks, cost and recovery of a chin implant, chin filler is a valuable way to strengthen the profile and positively change the character of the lower face.

The Art of Profiloplasty: The Strategic Role of the Chin

Facial beauty comes not from perfect individual features but from their harmonious relationship. Profiloplasty looks at the profile as a whole and aims to balance the forehead, nose, lips and chin. As the end point and anchor of the profile, the chin has a decisive effect on balance and character.

A simple way to understand this balance is Ricketts’ E-line, an imaginary line from the nose tip to the most prominent point of the lower lip. In an ideal profile the chin just touches this line or sits slightly behind it. When it sits well behind it, this is called microgenia or retrogenia – a “receding” or “weak” chin. This affects the face in several ways. First, a receding chin makes the nose look bigger; many people seek rhinoplasty because of a disproportion that actually comes from the chin, and sometimes chin filler alone balances the profile without nose surgery. Second, a weak chin cannot support the soft tissue of the neck and under the chin, so the area looks fuller, the neck-chin angle is lost and the jawline blurs; a strong chin stretches this tissue forward and sharpens the jawline. Third, the chin supports the lower lip; a receding chin can make it look turned in and unsupported.

Many patients at our clinic say a weak chin makes them look indecisive or childlike. I see chin filler not just as adding volume but as recalibrating the proportions of the face, giving it a more mature, decisive and strong character.

Chin Filler

Chin filler is a non-surgical procedure in which dense hyaluronic acid fillers with high lifting capacity are injected to increase and improve the size, shape, length and projection of the chin (mental region), without an implant. It redefines the contour of the lower third of the face, strengthens the profile and improves symmetry.

Because the chin must imitate bone and give strong structural support, fillers with a higher G-prime – denser, firmer and with more lifting capacity – are chosen. They stay in place in the deep plane, do not spread and behave like a natural extension of the jawbone, for natural results. Chin filler can:

  • Increase projection: the most common aim, bringing a receding chin forward for an ideal profile.
  • Lengthen the chin: vertical injections beneath the chin can increase the height of a short lower face, helping round or square faces look more oval.
  • Shape and widen: in men, injections towards the sides can create a squarer, stronger chin; in women, a more pointed, feminine “V” shape.
  • Correct asymmetry, whether congenital or after injury.
  • Soften a cleft chin by filling the dimple for a smoother surface.
  • Fill the pre-jowl sulcus, the groove just in front of the jowls, for a straighter, uninterrupted jawline.

Treatment begins with a consultation analysing your facial structure, bone anatomy and goals. On the day, a local anaesthetic cream is applied. Injections are made with fine needles for deep injections on the bone and with blunt cannulas for more superficial shaping and safety. The filler is placed in small amounts, layer by layer, to sculpt the chin. It takes about 15–30 minutes and results are visible at once.

The Procedure and Techniques

The process begins with a consultation based on trust and transparency. I listen to your expectations and carry out a comprehensive analysis of your face from the front and in profile: vertical and horizontal proportions, balance between the lower, middle and upper face, and the nose-lip-chin relationship. The chin’s current projection, height, width and any asymmetry are noted. I then decide whether chin filler suits you, roughly how much filler is needed (usually between 1 ml and 4 ml) and which technique to use, and explain the possible results and limits.

On the day, photos are taken, the chin area is cleansed and disinfected, and a strong local anaesthetic cream is applied for about 20–30 minutes. Then:

  • Deep needle technique: used especially to add forward projection or strengthen bone structure. A fine needle passes vertically to the membrane covering the bone (periosteum) and filler is placed as a small bolus, so it behaves like an extension of the bone and gives strong support. Precise injections can also correct asymmetry or lengthen the chin.
  • Cannula shaping technique: blunt, flexible cannulas spread filler evenly over wider areas, shape the sides of the chin and fill areas such as the pre-jowl groove. Inserted through a single entry point under or beside the chin, the cannula minimises the risk of damaging vessels and nerves, and filler is left in linear threads for a smooth contour.

The best and most natural results often come from combining both, with final gentle shaping by hand. It takes 15 to 30 minutes, depending on the amount and technique.

Who Is It Suitable For, Results and Longevity

Chin filler suits almost any healthy adult unhappy with the proportions of their lower face and profile, women and men alike. Ideal candidates include people with:

  • A receding or weak chin (retrogenia): the most common and clear indication; added projection instantly improves the profile, makes the nose look smaller and reduces the look of a double chin.
  • An asymmetrical chin that is less developed on one side or shifted.
  • A short lower face, which vertical filler can lengthen for a more oval look.
  • Women with an undefined or square jaw who want a softer, more feminine “V-line”.
  • Men with a weak jaw who want a more masculine, angular chin.
  • Age-related volume loss, as bone resorption makes the chin recede and lose its supporting role.

For significant skeletal or orthodontic problems (such as a severely receding lower jaw), surgery (genioplasty or orthognathic surgery) may be the right solution; filler is ideal as camouflage for mild to moderate cases.

Results are visible immediately. Mild swelling, redness or tenderness is normal and usually passes within a few days; the final result appears after about 1–2 weeks. Longevity depends on the filler’s density (G-prime), cross-linking, the amount injected and your metabolism. Dense fillers in a bony, relatively still area like the chin last longer than elsewhere: on average 12 to 24 months, after which the filler is safely broken down and the treatment can be repeated. With realistic expectations and an experienced physician, chin filler is a very safe procedure with high satisfaction that improves the overall harmony of the face.
