Cheek Lipolysis

Cheek lipolysis is an advanced aesthetic treatment that aims to reduce stubborn fat in the lower face, especially the cheeks, which can make the face look round, full or “baby-faced” – without surgery, using injections. A “V-line” or triangular face shape, with defined cheekbones and a sharp jawline, is widely seen as elegant and attractive, but genetics, weight changes or age can keep the cheek fat (particularly the buccal fat pad) prominent.

The traditional solution is buccal fat removal (bichectomy), in which the buccal fat pad is removed surgically through the mouth. Cheek lipolysis offers a modern, minimally invasive alternative: lipolytic (fat-dissolving) solutions that break down fat cell membranes are injected with very fine needles directly into the target fat in the cheeks. By permanently reducing the number of fat cells, it gives a controlled slimming and shaping of the lower face, helping the cheekbones stand out and creating a more elegant facial oval. At our clinic in Beşiktaş, Istanbul, I plan each treatment around the patient’s individual facial structure.

What Is Cheek Lipolysis and How Does It Work?

Cheek lipolysis targets excess fat in the cheeks – the buccal (Bichat’s) fat pad or subcutaneous fat – that makes the lower third of the face look full. It is designed for people with naturally full cheeks that do not respond to weight loss, as a less invasive alternative to bichectomy.

It is based on chemical lipolysis: fat cell (adipocyte) membranes are irreversibly damaged by chemical agents. The most commonly used combination is deoxycholic acid (DC) and phosphatidylcholine (PPC). Deoxycholic acid is a bile acid found naturally in the body that helps digest fats; its synthetic form acts like a detergent on fat cell membranes, causing the cells to break down (adipocyte lysis or cytolysis). The stored fat (triglycerides) and other cell contents are released, and phosphatidylcholine helps emulsify the released fat into smaller droplets that the body can process more easily.

After injection, the body’s natural cleaning and repair process begins. Macrophages engulf the debris and remove it through the lymphatic system to the liver for processing and elimination. This takes several weeks, and the final result depends on it. The controlled inflammatory response also stimulates fibroblasts to produce new collagen (neocollagenesis), which helps tighten the skin and prevent laxity after fat reduction, for a clearer facial contour. Success depends on choosing the right patient and placing injections at the correct anatomical points, depth and amount, so it should be done by an experienced physician with detailed knowledge of facial anatomy.

Who Is Cheek Lipolysis Ideal For?

Not everyone with a round face is a suitable candidate. Facial fullness is not always due to fat: a wide jawbone or prominent masseter (chewing) muscles can create the same look. That is why I carry out a detailed facial analysis first. The ideal candidate:

  • is in good general health and at or near a normal weight, with stubborn, localised fat in the lower face and cheeks that does not respond to diet and exercise – a structural fullness rather than general excess weight, confirmed on examination to be from the buccal fat pad or subcutaneous fat;
  • has good to moderate skin elasticity, so the skin can adapt to the new contour once the fat is reduced;
  • has realistic expectations: the treatment gives a natural, more contoured slimming, not a dramatic change of face shape.

If cheek fullness is due to an enlarged masseter muscle, the right treatment is masseter Botox, not lipolysis; if it is due to a wide bone structure, injectable treatments will not help. Where skin elasticity is very poor and sagging is advanced, losing fat may make sagging more visible, so combining lipolysis with skin-tightening treatments (thread lifting, radiofrequency or ultrasound) or considering a facelift may be more appropriate. The treatment is not suitable during pregnancy or breastfeeding, with active skin infection, bleeding disorders, or soya allergy (for products containing phosphatidylcholine).

A Treatment That Requires Skill and Precision

Because the cheeks are highly visible and anatomically delicate – with important nerves (especially the marginal mandibular branch of the facial nerve) and blood vessels passing through – cheek lipolysis requires detailed anatomical knowledge, experience and an aesthetic eye.

Consultation: Facial Analysis and Strategy

I listen to your goals and why the look of your cheeks bothers you, then examine your face at rest and in motion. The thickness, position and borders of the cheek fat pads are assessed by touch, along with skin elasticity, muscles and bone structure, to identify whether the fullness comes from fat, muscle or bone. If you are suitable, a treatment map is prepared with the exact injection points, the amount at each point and the depth. Usually 2–3 sessions, 4–6 weeks apart, are planned for the best results. You are told in detail about what to expect afterwards, especially swelling in the first few days.

The Procedure
  1. Preparation: the face is cleansed and the cheek areas disinfected; a strong topical anaesthetic cream is applied for about 30 minutes, sometimes with a cold compress.
  2. Marking: the planned injection points are marked with a sterile pen to ensure symmetry and accurate placement.
  3. Injection: the solution is injected slowly and carefully with very fine (usually 30G or 32G) needles, a very small amount at each point, at the correct depth within the fat and at a safe distance from important nerves and vessels. Both cheeks take about 15–20 minutes.
  4. Finishing: the skin is cleaned again and a cold compress applied straight away to minimise swelling and bruising.
Recovery and Results

You can leave the clinic straight away, but expect noticeable swelling, especially in the first 2–3 days – a normal inflammatory response showing the treatment is working – with possible tenderness, redness and bruising. To help:

  • apply cold compresses regularly for the first 48 hours (for example 15 minutes every hour);
  • sleep with your head slightly raised on a few pillows;
  • avoid strenuous exercise, saunas and hammams for the first 24–48 hours;
  • use any anti-inflammatory creams or medicines your physician recommends.

Swelling usually subsides considerably within a week. The final results take time, as the body clears the destroyed fat cells and builds new collagen: slimming is usually noticed after 3–4 weeks, and the clearest result about 6–8 weeks after the last session – more elegant cheeks, more defined cheekbones and a more balanced facial contour. Results are long-lasting as long as you do not gain significant weight, and vary between individuals.
