Double chin lipolysis is a modern, non-surgical aesthetic treatment that aims to reduce stubborn localised fat under the chin – the “double chin” – by injecting special fat-dissolving solutions. The area under the chin affects the whole face and profile, and fullness there can appear regardless of weight because of genetics, ageing or hormonal factors, making a person look older or heavier than they are.
The treatment uses lipolytic (fat-dissolving) agents that break down fat cell membranes so the fat can be cleared by the body naturally. Substances such as deoxycholic acid – a bile acid also found naturally in the body – or phosphatidylcholine are injected with fine needles directly into the fat under the chin. This permanently destroys fat cells (adipocytes), and their contents are cleared by the lymphatic system. The result is a lasting reduction in the number of fat cells in the area, for a more defined, sharper and younger jawline. At our clinic in Beşiktaş, Istanbul, I plan each treatment individually based on facial anatomy and the patient’s expectations.
What Is Double Chin Lipolysis and How Does It Work?
Double chin lipolysis is an injection-based alternative to surgical liposuction for excess fat in the submental area (under the chin) that resists diet and exercise. Unlike the temporary shrinking of fat cells that happens with exercise, injection lipolysis has a cytolytic effect – it destroys the cells directly – so the number of fat cells in the treated area is permanently reduced.
The most widely used active ingredient is deoxycholic acid, which is widely used for reducing submental fat. In the body, deoxycholic acid is a natural bile acid that helps digest dietary fats; the form used in aesthetics is synthetic. Injected into fat, it acts like a detergent on fat cell membranes, causing the cells to break down (adipocyte lysis). The released fat and cell debris are then removed by the body’s cleaning cells (macrophages) through the lymphatic system over weeks to a few months.
This controlled inflammatory and clearing process also activates fibroblasts, which produce new collagen (neocollagenesis) in the area, helping the skin tighten. This is why the treatment can both reduce fat and help define the jawline. Another lipolytic agent, phosphatidylcholine (PPC), derived from soya, is often combined with deoxycholic acid; it emulsifies fats into smaller droplets, and the combination can break down fat cell membranes more effectively. I assess the structure of your double chin, the amount of fat and your skin’s elasticity to choose the most suitable agent and technique.
Who Is Double Chin Lipolysis Suitable For?
It is not the right solution for every double chin, so correct patient selection is essential. The ideal candidate is generally healthy, at or near a normal weight, with a moderate amount of fat under the chin that resists diet and exercise because of genetics or anatomy – the key point is that the problem is mainly localised fat. If fullness is caused by advanced skin sagging, loosening of the neck muscles (platysma) or obesity, lipolysis alone will not be enough. If skin elasticity is very poor, the skin may look looser once the fat is reduced; in such cases a combined protocol with skin-tightening treatments (thread lifting, radiofrequency or ultrasound) or surgery (neck lift) may be more suitable.
- Younger and middle-aged patients are often the best candidates, as their skin still has good elasticity and can adapt to the new contour.
- People with a structural double chin who have more fat cells under the chin regardless of weight benefit from a lasting reduction in fat cell number.
- People who wish to avoid surgery – anaesthesia, incisions, stitches and long recovery – find it a practical alternative.
It is not given during pregnancy or breastfeeding, or where there is an active infection or skin condition in the area. Care and physician approval are needed for people with bleeding disorders or taking blood thinners, and products containing phosphatidylcholine are not suitable for those allergic to soya. Double chin lipolysis is a body-contouring method, not a weight-loss method.
What Happens During a Session?
Consultation and Planning
I listen to your concerns and expectations, then examine the area under your chin: the amount, density and distribution of fat, skin quality and elasticity and the visibility of the platysma bands, to determine whether the problem is mainly fat. If you are a suitable candidate, a personal plan sets out which agent will be used, the injection points and depth, and the number and spacing of sessions – usually 2 to 4 sessions, 4–6 weeks apart. Possible side effects (swelling, bruising, tenderness) and recovery are explained openly.
The Procedure
- Preparation: the area is cleaned with an antiseptic, and a guide grid is drawn on the skin so the solution is distributed evenly and accurately. A topical anaesthetic cream is applied for about 20–30 minutes, or the area is numbed with a cold pack.
- Injection: the lipolytic solution is injected into the fat with very fine needles, a small amount at each point, carefully avoiding important nerves and blood vessels. The number of injections depends on the size of the area; the injections usually take no more than 10–15 minutes.
- Finishing: the area is cleaned again, gently massaged if needed, and a cold compress applied to reduce swelling and discomfort.
Recovery and Results
You can return to daily life straight away, but there is a healing phase. Swelling, redness, tenderness and bruising are common and are part of the normal inflammatory reaction as the fat cells break down. Swelling is usually most noticeable in the first few days and settles gradually over 1–2 weeks; bruises usually fade within a week. Regular cold compresses, sleeping with your head raised and avoiding strenuous exercise for a few days help recovery.
Results are not immediate, because the body needs time to clear the destroyed fat cells and produce new collagen. The first changes are usually noticed after 3–4 weeks, and the final result about 6–8 weeks after the last session: a slimmer area under the chin, a sharper jawline and a more elegant profile. Because the treated fat cells are destroyed, results are long-lasting as long as you do not gain significant weight. Results vary between individuals.
