Mouth Area Anti-Ageing

Mouth area anti-ageing is a holistic aesthetic approach for the perioral area – the lips and surrounding skin – which is at the centre of our expression when we speak and smile but often shows ageing in the most noticeable ways. Vertical lines above the lip (barcode lines), downward grooves from the corners of the mouth (marionette lines) and loss of lip volume can make a person look not only older but also unhappy, irritated or tired.

Mouth area anti-ageing addresses all of these with personalised, combined protocols: Botox to balance muscle movement, filler to replace lost volume and mesotherapy to improve skin quality. At our clinic in Beşiktaş, Istanbul, I believe every smile is unique, so the aim is the most natural, elegant result – removing the marks of time and lifestyle without changing your natural expression.

What Overshadows a Smile: Why Does the Mouth Area Age?

The perioral area is particularly vulnerable to ageing because of its specific anatomy and function:

  • Constant muscle activity: the circular orbicularis oris muscle around the mouth works like a drawstring every time we talk, eat, smile, drink or smoke. Young, elastic skin springs back, but as collagen and elastin decline, fine vertical “barcode” or “smoker’s lines” form, especially on the upper lip, where lipstick tends to bleed.
  • Thin skin and volume loss: the skin here is thinner, with less fat beneath. With age, deep fat compartments and the jawbone also lose volume, so tissues around the mouth sag – the main cause of marionette lines, which give a sad, puppet-like look. The lips themselves become thinner, turn inwards and lose fullness.
  • Changes in teeth and jaw: tooth wear or loss reduces the height of the jawbone, removing support from beneath the lips and deepening lines.
  • Sun damage: UV rays break down collagen and elastin in this delicate area, causing wrinkles earlier.
  • Smoking, which both repeatedly puckers the lips and, through its toxins, disrupts circulation and increases collagen breakdown.

Mouth Area Anti-Ageing

Because the area is very mobile and combines several ageing problems, it needs a multi-faceted strategy rather than a single treatment – like fitting together the pieces of a puzzle. I follow the principles of “less but better” and “natural” to create the right combination for each patient:

  1. Controlling dynamic wrinkles (barcode lines): without controlling the puckering movement of the orbicularis oris, filling the lines alone does not give a lasting, natural result. The first step is usually very low-dose Botox (“micro-Botox”) at specific points above the lip, gently relaxing excessive puckering so lines do not deepen and new ones are less likely to form. The dose must be adjusted very carefully so speaking and smiling are not affected.
  2. Static lines and volume loss: fine-line filler – a very thin, soft, fluid, low-density hyaluronic acid – is injected very superficially into barcode lines with a cannula or very fine needle, simply raising the base of the line to smooth the skin. For marionette lines, which usually stem from mid-face sagging, structural filler in the cheeks and jawline first creates a lifting effect, then a more supportive filler can be placed in any remaining groove. Lip filler, aiming for a natural look, can restore gentle volume and contour to thinning lips, making the whole area look younger.
  3. Improving skin quality: mesotherapy and skin boosters with pure hyaluronic acid, peptides, vitamins and amino acids nourish, hydrate and repair the skin. Energy-based devices such as fractional lasers or radiofrequency renew the surface and stimulate collagen in deeper layers, which is especially effective for barcode lines and tightening.

A Multi-Step Treatment for Barcode (Smoker’s) Lines

The vertical lines between the nose and the red border of the upper lip are known medically as perioral rhytides and commonly as barcode or smoker’s lines. They make the face look older and allow lipstick to bleed into them. Effective treatment works on several fronts:

  1. Control muscle activity (Botox): very low doses at specific points of the upper lip (“micro” or “baby Botox”) gently relax the muscle – not paralysing it, just preventing it from creasing the skin excessively. Doses and points must be set very precisely by an experienced physician, otherwise there may be slight difficulty with speaking or drinking through a straw. Done correctly, it stops lines deepening and prepares the ground for other treatments.
  2. Fill the line (filler): for static lines visible at rest, a very fine, soft, fluid, lightly cross-linked hyaluronic acid filler is placed very superficially at the base of each line with a cannula or very fine needle, gently raising the groove to smooth the skin rather than plumping the area (the “blanching” technique).
  3. Boost skin quality and collagen: barcode lines also reflect thinning skin and collagen loss. Mesotherapy and skin boosters hydrate and stimulate new collagen. Energy-based devices – fractional laser (erbium or CO2) or fractional radiofrequency (gold needle) – are among the most effective options, creating microscopic columns of controlled injury that the body repairs with intensive new collagen, resurfacing the skin and tightening it from beneath.

The plan depends on line depth, skin type and age; sometimes the best result comes from starting with Botox, followed by a few sessions of laser or radiofrequency and finally a very light touch of filler for any remaining fine lines.

Solutions for Marionette Lines and Downturned Corners of the Mouth

The grooves running from the corners of the mouth towards the chin can give the face a fixed look of sadness, unhappiness and fatigue. They are named after the lines on a ventriloquist’s puppet (marionette). They are not just deepened lines but the result of structural changes in the mid and lower face:

  • Mid-face volume loss and sagging: as cheek fat pads shrink and bone support decreases, cheek tissue slides down and gathers at the corners of the mouth, creating a groove and shadow.
  • Loss of jawline support: receding jawbone and weakening fat and connective tissue along the jawline accentuate the lower border of the line.
  • Activity of the depressor anguli oris (DAO) muscle, which pulls the corners of the mouth down and can become more active with age.

I take a “facial architecture” approach:

  1. Lift the face: treatment usually starts by correcting mid-face sagging with denser, structural hyaluronic acid filler in the cheeks and cheekbones, lifting the cheek tissue up and back and reducing pressure on the marionette line. Filler along the jawline supports the lower part of the line. Often this indirect approach alone brings clear improvement.
  2. Relax the muscle (Botox): a very small dose of Botox in the DAO muscle reduces its downward pull so the corners of the mouth lift slightly into a neutral position – a simple touch that is very effective against a sad, tired expression.
  3. Fill the line directly: if some depth remains, a medium-density hyaluronic acid filler that integrates well with tissue is injected into the groove, usually with a cannula for safety and even spread.
  4. Improve skin quality: mesotherapy, skin boosters or biological lifting can be added to support firmness and skin health in the long term.

This holistic approach addresses the problem at its source rather than just the surface, restoring a positive, dynamic and youthful expression.
